Blood pressure in most people follows a predictable daily arc: it rises sharply in the early morning hours — often between 6 a.m. and noon — dips during the afternoon, and falls to its lowest point overnight. This morning surge is associated with an elevated risk of heart attack and stroke.

For years, the default recommendation was to take antihypertensive medications first thing in the morning. However, a large Spanish clinical trial known as the Hygia Chronotherapy Trial found that patients who took their blood pressure medications at bedtime had significantly better cardiovascular outcomes than those who took them in the morning. Statins: Why Evening Is Generally Preferred

Longer-acting agents like atorvastatin and rosuvastatin are more flexible, with studies suggesting their efficacy is less dependent on timing.
